In order to register at Gallatin High School, the following documents MUST be presented at the time of registration:

  • Birth Certificate
  • Custody Papers (If applicable.  Student must reside in Sumner County with the PRIMARY RESIDENTIAL CUSTODIAL PARENT OR GUARDIAN.)
  • Current Immunization Records (on TN form if coming from out of state or from private school) https://sumnercountytn.gov/departments/health-department/
  • Current physical exam (within the past year)
  • Photo ID (to identify the parent/guardian enrolling student)
  • Two (2) Proofs of Residence from any of the following options
    • Legal document/lease showing proof of residency for a specific address (Notary public or typed/hand written letters from the land owner are not accepted.)
    • Current invoice/bill (electric, gas, water, landline telephone) assigned to a specific address AND the primary custodial parent/guardian. (Cell phone bill is NOT accepted.)
    • If you are unable to provide the above information, you should ask about the Residency Affidavit Form and process.
  • Records (including withdrawal form, transcript, final report card, IEPs, 504s, etc.) from student’s previous school are always helpful.
